Sub:- Request for obtaining information under section 6 of the Right to Information Act 2005—Reg.

Please supply the following information on the address cited as above. This is an application in accordance with the provision of section 6 of the right to information act 2005.

Information and relevant documents seeked for related to executives including the I.T.S. officer posted in the office of the WTR, Jabalpur are as below:-

1. Please supply information regarding sanctioning of House building advance to executives including the I.T.S. officer posted in the office of the WTR Jabalpur along with the sanctioned and disbursed amount to them from 01-10-2000 to 31-03-2008.
2. The name and designation of those have been sanctioned the House building advance may also be supplied along with the sanctioned amount of each case.
3. Please supply information in respect of those who have obtained full sanctioned amount along with the date and amount of installment last disbursed.
4. Please supply the copy of the sanctioned map, for construction of house on which the house building advance was sanctioned,by the Municipal Corporation/Nagar Palika Parishad etc.
5. Please supply the completion certificate in respect of those who have completed the construction of their house for which the HBA was sanctioned. In case the completion certificate has not been given by the officer obtained HBA for, what action has been taken by the competent authority in this regard? The copy of the action taken report may please be supplied. If no action, the reason thereof.
6. Please supply any other information available with the office of the CGM, WTR, Mumbai in respect of those who have received the full amount of House building advance and have not constructed the House yet. Please also supply a copy of action taken report in case of defaulter if any. If no action has been initiated against defaulters please intimate the reason thereof.
7. Please also supply the name of tose officers who have not completed the construction of house in 18 months from the date of the drawal of the 1st installment or within the extended time. If any action has been taken in this regard? If yes, please supply a copy of the action taken report. If not, the reason thereof.
8. Please supply the name of officers obtained House building advance and have insured the house with the life insurance Corporation of India. Please also inform if any action has been initiated against those who have not insured the constructed house? If yes, the copy of the action taken report may be supplied. If no, the reason thereof.
9. All 8 paras relates for the period from 01-10-2000 to 31-03-2008.
10. Please find herewith enclosed the IPO No.67E-178097 as requisite fee of Rs.10/- payable to the Account Officer % CGM, WTR, Mumbai.
11. The additional amount to be paid by me for photo stat copy etc. may please be intimated to me on my mobile No. 9425124499 which will be deposited in cash with the A.O. % The CGM, WTR, Mumbai, to avoid delay, and the receipt would be produced immediately
